Upload up to 10 files, up to 1 GB each: video, photos, audio, PDFs or documents. Keep your original evidence and avoid editing the original recording.

- Incident date or date range
- Description of the crime
- Your interaction with police
- Any police follow-up
- Anything else you want us to know
- Video or other supporting evidence
Uploads are optional. You can submit the summary first and email additional information later.

We review the information and contact you by email about possible next steps. Submitting this form does not authorize police outreach or publication. Any authorization is handled separately.
